How We Support Clients Every client’s support is overseen by our Positive Behavioural Support (PBS) Specialist, Registered Manager and Registered Nurses – ensuring care is personalised and effective. Our multidisciplinary team works closely with the individual, their family, guardians, and healthcare professionals to provide consistent, tailored support that aligns with the client’s needs, preferences, and goals. ABOUT US Safety and Assessment A comprehensive risk and needs assessment is conducted to ensure safety and stability from the outset. Assessments ensure we understand support requirements, personal aspirations and potential challenges. To get a full picture of how we can best support an individual, we collaborate with the individual and their support network – family, guardians and healthcare professionals. Part of the assessment reviews the client’s environment to mitigate risk and implement a plan to provide a safe space that promotes empowerment and independence. Digital Care Plan Tool Clients’ families and guardians receive real-time visibility of outcomes, reviews, care plan changes and goal progress. Service Improvement Our Service Improvement Manager closely monitors our quality of care through a benchmarking system, which provides detailed reports on the standard of support services. Skilled and Experienced Staff Staff are trained to a high level, providing them with training to meet the needs of the individual they support. Some of our nurse-led training includes, ligature awareness and protocols, PBS techniques, medication management, and basic life support. Through our Meet and Greet Service, staff are carefully matched with clients – ensuring needs, personality and preferences can be supported to a high standard. We provide ongoing training and professional development to maintain the highest standard of care. LEARN MORE Holistic Wellbeing Support Using our PBS approach, Support Workers employ evidence-based strategies to prevent crises and promote wellbeing – helping clients to live happy and healthy lives. Clients are encouraged to engage in social activities, build relationships and enjoy community participation. If support is required for an activity, the client will be given the choice of which team member they want to support them, and we schedule their support accordingly. Support extends beyond mental health, including physical health, daily routines, life skills, employment, education and achieving personal goals.
