Every support package is tailored, outcome-focused, and built around the person, not just the condition. GET IN TOUCH Supporting Case Managers We work in close partnership with the individual, Case Managers, families, and healthcare professionals to build support packages that are robust, responsive, and person-centred. Each package is supported by comprehensive training, strong clinical governance, and ongoing supervision, ensuring safety, consistency, and excellent outcomes. We are known for not only delivering person-centred support, but we will also provide you with: Transparent, collaborative communication 48-hour turnaround of care packages Timely reporting and clinical oversight Person-led goals that align with rehabilitation or independence pathways SUPPORT FOR YOUR CLIENTS Providing Transparency and Real-Time Visibility We use a live digital care plan platform which you will have access to, along with your clients’ next of kin. Our digital tool makes the documentation process more efficient – which means more time spent focused on your client. Our system enables you to have oversight of: Outcomes and Goal Tracking Care Delivery Daily Activity Logs Real-Time Oversight Quality Assurance Responsive Support you can Rely On Our dedicated Mobilisation Support Managers provide a seamless and responsive experience for Case Managers and their clients. Upon receiving a referral, our Mobilisation team will arrange an initial meeting with you, the Case Manager to understand your client’s needs, circumstances, and goals. We then meet the client in-person to complete a comprehensive assessment – always led by a person-centred approach. If we’re confident we can meet your client’s requirements and help them live a safe, fulfilling life, we will begin the agreement process. Once confirmed, we start building the support package – creating the care plan, risk assessment, and recruiting suitable Support Workers. Where appropriate, clients are invited to take part in Support Worker interviews, helping them choose someone who’s the right fit. Our Meet and Greet service follows to ensure a strong, trusting relationship from the outset. All new clients receive weekly visits from their Mobilisation Support Manager, ensuring that every aspect of care is meeting expectations before the package is formally handed over to one of our experienced Case Officers.
