We collaborate closely with Case Managers, Solicitors, health and social care professionals, and family members to build a strong understanding of the client’s needs and wishes. Our digital care plan platform provides Case Managers and next of kin with real-time updates on progress, goals, routines, and support outcomes.

Tailored Support

We are committed to delivering person-centred care by involving individuals in their care planning from the onset.
Through collaboration with the individual and their support network, care plans are produced and reflects the client’s unique needs, preferences and desired outcomes.
Our services are delivered in-line with the Care Act 2014, the Mental Capacity Act 2005 and the REACH standards which follow the principles of the personalisation agenda.
We use inclusive communication methods, such as PECS, social stories and Makaton. This ensures everyone can actively participate in shaping their care and support.
Our in-house nurse-led training ensures Support Workers are equipped with the skills and competencies needed for each client’s specific needs.
Client goals and milestones are continuously monitored through our digital platform, supporting progress and promoting a sense of achievement.

Clinical and Behavioural Support

Our nurse-led training programme enables team members to gain a wide range of clinical care skills, including acquired brain injury support, tracheostomy care, and PEG feeding.
Our Positive Behaviour Support (PBS) Specialist creates bespoke PBS support plans for each client and provides Support Workers with evidence-based strategies for behavioural interventions for complex needs.
We assist clients attending medical appointments, including occupational therapy, speech and language therapy and physiotherapy.
