Integrated Care Our multidisciplinary team use the digital care planning system, iplanit, to maintain goal-oriented care plans. 24-Hour Complex Care Packages Our team provides complex care for high-acuity needs within the client’s own home. Unlike a typical supported living service, we provide continuity of care through 24/7 support packages with shifts of six hours or more.
