GET IN TOUCH Nursing Oversight of Support Workers Support Workers delivering clinical care at home require comprehensive training that goes beyond standard care qualifications. This includes condition-specific clinical training, competency assessments, and ongoing supervision from Registered Nurses. While Nurses may not be present for every shift, they will have involvement in every clinical care packages. Their support can range from carrying out clinical interventions to providing clinical supervision and competency assessments to the creation and monitoring of care plans. DISCOVER CLINICAL CARE Transitioning from Hospital to Home While support can initially be given within hospital setting, transitions from hospital to home are the main goal. This critical period of clinical home care is also known as discharge to assess and hospital-to-home-mobilisation, requiring careful planning to ensure continuity of care. Key considerations must be taken into account for a successful transition, if it is deemed appropriate to deliver home-based clinical care. Assessments & Planning Before discharge all clinical needs, equipment requirements and support arrangements necessary will be assessed to ensure safe support at home. Co-ordination with Multidisciplinary Teams Relationships with GPs, District Nurses, consultants, and health professionals are maintained.
