We are a CQC-rated Good provider and BABICM members who provide real-time data via our digital care planning tool, iplanit. It enables us to give Case Managers and health and social care professionals real-time access to daily notes, care plans, and outcomes, so you always know what’s happening without having to chase. Through iplanit, Case Managers, Deputies, families and other professionals can view daily notes and logs, care plans, support tasks completed, activities undertaken, and outcomes achieved in real time, not just at review points. We believe open access to this information isn’t just good practice, it’s what genuine accountability looks like. Our Specialist Support Can Include: Clinical Care Our Clinical Support Workers receive training tailored to the needs of the individual. This could include tracheostomy care, stoma care or PEG feeding. Medication Management As part of our mandatory training, Support Teams receive medication training, incorporating buccal administration and epilepsy awareness where required Positive Behaviour Support (PBS) For individuals who display behaviours of concern, our PBS Lead develops a personalised PBS plan and provides tailored training to Support Teams and families to implement it consistently and confidently. Daily Routines & Independence Tailored support to help individuals maintain routines, build confidence and develop greater independence. Communication & Interaction Where required, our Support Workers are trained in a range of communication methods, including PECS, eye gaze and Makaton, to support each individual’s communication needs and preferences. Goal-Focused Rehabilitation Our personalised care plans consider each individual’s care needs, preferences and personal goals, providing a clear framework for delivering consistent, person-centred support. Community Integration We support individuals to access their communities, build meaningful relationships and take part in activities that reflect their interests, goals and aspirations. Collaborative Care We work collaboratively with individuals, their families and health and social care professionals to provide joined-up support that promotes rehabilitation, independence and long-term progress. GET IN TOUCH Positive Behavioural Support For individuals with an acquired brain injury who experience behaviours of concern, we provide specialist Positive Behavioural Support (PBS) delivered by qualified PBS practitioners, who produce PBS plans and provide training to Support Workers and families to promote consistent tailored support. Our approach goes beyond the initial intervention. Where many providers may step back once a crisis has passed, we continue to work proactively with the client to sustain positive outcomes, reduce risk over time, and help prevent behaviours of concern from re-emerging.
